The Federation of Small Businesses (FSB) would like to use your member story to help publicise and promote what we do and the services we provide. This information will be used to create a member story on FSB’s website, social media, press releases, and print and digital adverts. By completing this form, you agree for the information given to be used to contribute to these member stories.

I consent to FSB arranging a photographer to visit me at my business premises to take photos to go alongside my member story.

I consent to FSB arranging a videographer to visit me at my business premises to record me (in a simple interview style recording) to go alongside my member story.

I consent to FSB calling me to conduct a telephone interview as part of my member story.

Your consent will be valid for a maximum of two years or until the videos and/or photographs are no longer required. After this period, your images will be securely deleted from our systems.

You may withdraw your consent and request FSB to stop using your images in new materials by emailing [email protected], however removal of your image from existing published material may not be possible.
